Emergency Medical Form
This form lists information emergency personal may need to know in case you would be involved in an accident or medical emergency and you’re unable to respond to their questions. You can print this form and fill in your information. Keep this form somewhere on your bike in a sealed envelope marked Emergency Medical Information. Next time you’re out riding with your buddy or in a group ride let someone know where it is located on your bike. They can let the emergency personal know where it’s at and/or someone will be able get it for them. You should also do this for any passenger that is riding with you.
